Understanding Practice Integration in the U.S. Medical Eye Care Industry [Draft]
Abstract: Health care services in the U.S. are increasingly delivered by multi-specialty practices employing physicians with different levels of qualification. This paper fills the gap in our understanding of the integrated practice mode and studies the determinants of physician participation in U.S. ophthalmology. Using detailed administrative data on individual level patient visits and physician characteristics, I build and estimate a structural model of physician participation, location, and type of practice choice and patient choice of physicians. My findings show that patients strongly prefer multi-specialty practices over single-specialty practices for their more serious health care needs (surgeries), and that large geographic variation in patient preferences for type of practice in routine services negatively correlates with physician availability. I conduct counterfactual simulations to study how market composition of practices and total spending shares at each type of practice can change based on projections of continued physician workforce decline and patient demand increase in the industry. I find such changes lead to heterogeneous responses across different geographic regions but sizable increase in importance of multi-specialty practices in markets with current high availability of physicians and low patient preference for multi-specialty practices.
